Hi David, I don't have any experience of this (or much else for that matter) but have you considered sedating the fish for a proper examination of his mouth/throat to see if there's anything lodged further in? It sounds like a physical issue rather than one caused by the usual culprits but I'm guess...

John, did you say in another thread that your filter is pump fed from the bd via the 4" pipe or are you gravity fed to the filter and use a pump to return?
